Empleo de la embolización renal transarterial selectiva en pacientes con masas renales

Resumen

  • Introducción: la embolización renal arterial selectiva (ERAS), como alternativa terapéutica, mínimamente invasiva, se emplea en diferentes condiciones patológicas.

    Objetivo: describir los resultados del empleo de la ERAS en las masas renales.

    Material y método: estudio descriptivo y retrospectivo en 36 pacientes, en los que se realizaron 44 ERAS, en el Hospital Clínico Quirúrgico Hermanos Ameijeiras y el Centro Nacional de Cirugía de Mínimo Acceso, 2019–2022. Se aplicó estadística descriptiva.

    Resultados: la edad media fue 55,3 años, predominaron los pacientes con comorbilidades (86,11 %), ECOG 0 (83,33 %). Las indicaciones más frecuentes fueron: paliativa, en pacientes con carcinoma renal con contraindicación para cirugía, o preoperatoria para disminuir sangrado intraoperatorio (35 %, respectivamente); la mayoría eran masas malignas (75 %) y los pacientes tenían estadio IV (51,85 %). El material embolizante más empleado fue el PVA (65,91 %). Alcanzaron embolización total 90,91 % de los pacientes y se logró el éxito esperado en 88,81 %, el cual no se asoció ni a las indicaciones ni al tamaño tumoral (p>0,05). El síndrome posembolización fue la complicación más frecuente (25 %).

    Conclusiones: la ERAS fue exitosa y segura en el tratamiento de las masas renales en que se empleó, generalmente se requirió una sola embolización, predominó la indicación paliativa en casos con CCR, en los que la cirugía no fue posible y se logró el infarto total de la masa en la mayoría.
