Introduction: Congenital ventral penile curvature is an uncommon developmental anomaly, with an estimated prevalence of 4–10% among congenital penile malformations, associated with ventral fibrous tissue or structural alterations of the tunica albuginea (chordee), leading to functional and psychosocial impairment.
Case presentation: A 19-year-old male presented with penile deformity since birth. Physical examination revealed a ventral penile curvature of approximately 90° during erection, located at the mid-distal shaft, with orthotopic urethral meatus and hypoplasia of the corpus spongiosum. A three-stage surgical reconstruction was performed. Surgical correction was achieved through a staged reconstructive approach using oral mucosal grafting, with satisfactory correction of the curvature and no significant complications during follow-up.
Conclusions: Staged reconstruction represents an effective alternative for severe congenital ventral penile curvature with urethral involvement, allowing favorable functional and aesthetic outcomes.
