Introduction: Primary renal squamous cell carcinoma (RSCC) is a rare entity. It represents less than 1% of all kidney tumors. Frequently, it is related to chronic inflammatory processes of the renal urothelium, which condition a secondary metaplasia, with renal lithiasis being one of the most important risk factors. Its symptomatology is vague and not very specific, the diagnosis late, and with it, a reserved prognosis.
Clinical case: We report the case of a 59-year-old male patient, history of repeated urinary tract infec- tions, not renal lithiasis, who reports diffuse pain of moderate intensity in the left lumbar region, associated with macroscopic hematuria, fever and involuntary weight loss. Imaging studies revealed large left renal mass and heterogeneity. Open left radical nephrectomy is performed, with a histopathological report that confirms the diagnosis.
Conclusion: Primary squamous cell carcinoma of the kidney presents with nonspecific clinical manifestations and inconclusive imaging findings that delay the initiation of treatment. It should be suspected in patients with a long-standing history of kidney stones or any inflammatory condition associated with a complex renal mass. Diagnosis is histopathological. Nephrectomy is the primary therapeutic option in the early stages of the disease.
